歯周病およびインプラント周囲組織の疾患と状態に関する新分類

009©2018 American Academy of Periodontology and European Federation of Periodontology皆様へ　このたび，2017年に開催されました「歯周病およびインプラント周囲組織の疾患と状態に関する新分類」についてのワールドワークショップ会議録を発刊するにあたり，アメリカ歯周病学会（AAP）およびヨーロッパ歯周病連盟（EFP）を代表しご挨拶申し上げます．　歯周病学の分野においては，この20年間で大きな進歩がありました．それに伴い，1999年から基準となっている歯周病の分類体系を再検討する必要が生じてきました．疾患の進行，潜在的リスクファクター，患者さんごとの複雑な特質についての臨床医の理解を深めるため，これまで数多くの研究が行われてきました．歯周病の分類を見直し，最新化し，標準化することは決して容易なことではありません．そのことを認識したうえでこの大事業を成し遂げるため，私たちは世界中の歯周病に関する知見を集積しました．2017年11月，アメリカ，ヨーロッパ，アジア，オーストラリアの専門家がシカゴに一堂に会し，歯周組織の健康，歯肉疾患，歯周炎，全身疾患が関連する歯周組織の状態，インプラント周囲組織の疾患について討論し，定義づけを行いました．健康と疾患に焦点を当てた症例の定義を作成するとともに，将来必要とされる研究を確認しました．計画に3年，集中した討議に3日，170名の各国の専門家から得られた情報の精査に数か月を費やし，今ここに，世界中の臨床医にとって従来の分類よりも明快かつ利便性が向上した新分類を発表できることを誇りに思います．　今回の新分類では，歯肉の健康と歯肉歯槽粘膜の状態を明確にし，インプラント周囲組織の疾患についてはじめて言及しました．前回策定された疾患分類の中から，病理生物学的観点に基づく区別に関し，重複および説明が不足している箇所の大部分を削除し，これまで以上に診断の精度に重点を置きました．また，癌の分類同様，多角的なステージ分類とグレード分類体系を導入しました．「ステージ」の分類は，疾患の重症度とケース・マネジメントの複雑度に基づいており，「グレード」の分類は，進行度，悪化のリスク，全身健康への潜在的リスクといった生物学的特徴に基づいています．　歯周病のさまざまなステージを理解し診断するこの革新的な新分類は，歯周病と患者さんの状態やリスクファクターに基づき発現・推移する他の進行性疾患との類似性を強調しています．また，今後も蓄積される知見やエビデンスを反映できるよう，シンプルな体系をとっています．このたびの新分類は，世界中で普及・施行されることになるでしょう．ただ，さまざまな利害関係者がこれを受け入れ，自分たちのシステムに取り入れ，世界中で治療の向上を始めるには，時間を要するでしょう．　歯周病は現在，世界第6位の慢性疾患です．その罹患率の高さが物語っているのは，世界中の患者さんの健康を向上させるためには，私たちの分野の協力が不可欠だということです．2012年に開催された歯周炎と全身状態に関するはじめての合同ワークショップの成功を受け，この大事業に対するAAPとEFPの取り組みは順調でした．しかし，時間，人材，ワークショップ参加者の皆様，著者の皆様，レビューワーの皆様の協力に加え，AAPおよびEFPのリーダーシップ，スタッフ，有志による委員会の協力がなければ，この大事業を完遂することはできなかったでしょう．最後になりましたが，本誌の共同発刊にあたってのJournal of Periodontology，Journal of Clinical Periodontologyのエディターの皆様のご尽力に謝意を申し上げるとともに，本ワークショップ開催にあたり多大なるご支援を賜りました，アメリカ歯周病学会財団，コルゲート，ジョンソン・エンド・ジョンソン コンシューマー カンパニー，ガイストリッヒ バイオマテリアル，サンスター，プロクター・アンド・ギャンブル プロフェッショナル オーラル ヘルスの各教育パートナー・企業スポンサーの皆様にも心からの感謝を申し上げます．　本誌が，歯周病専門医の日々の診療と，この先何年もの患者さんの健康と笑顔に寄与することを祈念しております．アメリカ歯周病学会会長　Steven R.
